Job Summary
Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team. Perform services in accordance with the physician's orders and the established plan of care. Maintain a valid state driver license, automobile liability insurance, and dependable transportation in good working condition. Possess CPR certification and basic demonstrated technology skills, including operation of a mobile device. This role is available for PRN positions at Enhabit Home Health & Hospice, one of the nation's largest home-based care providers.
Required Qualifications
- Masters degree from a school of social work accredited by the council on social work education
- Certification or licensure in the state where they currently practice
- At least one year of social work experience in a health care setting
- Basic demonstrated technology skills, including operation of a mobile device
- Valid state driver license
- Automobile liability insurance as required by law
- Dependable transportation in good working condition
- Ability to safely drive an automobile in all types of weather conditions
- CPR certification
