Job Summary
Conduct individual and group psychotherapy sessions for patients in subacute or skilled nursing facilities. Assess and diagnose emotional and behavioral health conditions including depression, anxiety, adjustment disorders, and cognitive decline. Develop and implement evidence-based treatment plans tailored to the needs of each patient. Collaborate with facility staff, interdisciplinary teams, and family members to support patient care. Maintain accurate and timely clinical documentation using the clinician-designed electronic medical/health system (EHR). Monitor patient progress and adjust treatment plans as needed. Requires active license as an LMFT and in-person service delivery at assigned facilities (up to 45 minutes). Preferred: experience in subacute/geriatric settings, comfort with CEU reimbursement programs, and ongoing mentorship opportunities.
Required Qualifications
- Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in state applying for
- Effective oral and written communication in English
- Basic proficiency with technology, including electronic health records (EHR)
- Strong organizational and documentation skills, with attention to regulatory compliance
- Ability to provide in-person services at assigned facilities, up to 45 minutes
