Job Summary
Conduct individual and group psychotherapy sessions for patients in subacute or skilled nursing facilities. Assess, diagnose, and treat emotional and behavioral health conditions such as depression, anxiety, adjustment disorders, and cognitive decline. Develop and implement evidence-based treatment plans tailored to each patient’s needs. Collaborate with facility staff, interdisciplinary teams, and family members to support patient care. Maintain accurate and timely clinical documentation using our clinician-designed EMR system. Monitor patient progress and adjust treatment plans as needed. Requires active LMFT licensure and ability to provide in-person services at facilities within NJ, NY, PA, or MD. Preferred experience in geriatric/subacute settings and with anxiety, depression, and adjustment disorders; openness to mentorship and ongoing professional development.
Required Qualifications
- Active and unrestricted LMFT license in the state of practice
- Ability to provide in-person services at assigned facilities
- Proficiency with electronic health records (EHR)
- Strong communication and documentation skills
- English proficiency
- Vaccination and booster status may be required in some facilities
