Job Summary
Provide counseling to clients/residents in approximately 15 care facilities per month within a 60-mile radius, covering a mix of nursing, group, and long-term care settings with varying acuity levels. Daily travel is required, necessitating a valid driver's license and reliable vehicle. Work within individual teams comprised of LPNs, PMHNPs, PhDs, and LISWs while maintaining schedule flexibility and supported autonomy. Required Qualifications
- Daily travel required
- LISW or LISW-S
- Ohio license
- Valid driver's license and reliable vehicle (with an acceptable driving record)
