Job Summary
Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) to join Eventus WholeHealth’s multidisciplinary team in long-term care settings. Provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities. Responsibilities include assessing residents with consent, developing treatment plans to meet needs, delivering psychotherapy, documenting plans and sessions within 48 hours, coordinating with facility staff and primary care providers, maintaining professional communications, and adhering to HIPAA, regulatory guidelines, and Eventus WholeHealth policies. Requires MSW with current state license (LCSW or LISW) and knowledge of assessment/psychotherapy techniques and DSM. Strong skills in data gathering, evaluation, written/verbal communication, initiative, and collaboration with health-care teams.
Required Qualifications
- Master’s degree in social work (MSW)
- Current license to practice in the state (LCSW or LISW)
- Knowledge of assessment techniques, psychotherapy techniques, DSM (current edition)
- Ability to conduct group and individual therapy
- Ability to develop and follow treatment plans
- Compliance with HIPAA and confidentiality requirements
