Licensed Independent Social Worker

Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) to provide recovery-oriented rehabilitation services in long-term care settings (skilled nursing and assisted living). Responsibilities include assessing residents with consent, developing and following treatment plans to meet individual needs, communicating with primary care physicians and facility staff, documenting treatment plans and sessions within 48 hours, coordinating with Resident Care Coordinators, and maintaining open communication with facility staff. Must adhere to HIPAA and regulatory guidelines, and consult with clinical supervisor for unfamiliar requests. Knowledge and skills include assessment and psychotherapy techniques, DSM familiarity, arranging and evaluating care, written and verbal communication, initiative and discretion, and collaboration with the health care team.
