Job Summary
Provide recovery-oriented rehabilitation services in group or individual settings as part of a multidisciplinary team in skilled nursing and assisted living facilities. Conduct psychosocial assessments, develop and implement treatment plans, coordinate with facility staff and primary care, document sessions and updates within 48 hours, ensure HIPAA-compliant confidentiality, and uphold regulatory and ethical standards. Maintain effective communication with residents, families, and care teams; demonstrate initiative, judgment, and discretion; and respond calmly in emergencies while delivering high-quality clinical care.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state (LCSW/LISW)
