Job Summary
Provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities as a Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW). Conduct assessments with valid consent, develop and follow treatment plans, document psychotherapy sessions and updates within 48 hours, and communicate with facility staff, primary care physicians, and Eventus WholeHealth support staff. Maintain HIPAA compliance, adhere to regulatory guidelines, and collaborate with an interdisciplinary care team to deliver high-quality clinical care in long-term care settings.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state
- LCSW or LISW eligible/required
