Job Summary
Conduct psychosocial assessments, mental health treatment, and referrals to community resources for patients in collaboration with the medical care team. Deliver individual and family therapy, including walk-in patients, while providing consultation to Swing Bed, ALF, and Long Term Care facilities. Evaluate patient data to develop and implement care plans, ensuring cultural competency across life stages from children to the elderly. Collaborate with the Utilization Management Coordinator and healthcare teams to address behavioral health needs, maintain supply of resource materials, and act on performance improvement issues identified during CQI meetings. Maintain confidentiality, prepare reports and case records, and participate in departmental meetings as assigned.
Required Qualifications
- Master's Degree in Social Work (MSW)
- Current licensure as a Licensed Independent Clinical Social Worker
- Two (2) or more years of previous experience in all aspects of social service, with an emphasis in psychiatric work
- Current certification/training in the safe management of disruptive and assaultive behavior
- Knowledge of cultural competency and treatment throughout life stages from children to elderly
- Familiarity with medical diagnosis and terminology
- Ability to communicate with physician regarding behavioral health needs of the patient
- Maintains current knowledge of social, medical and psychiatric services available within the community
- Ability to be flexible, organized and function under stressful situations
