Licensed Fee for Service Social Worker - Hybrid

Clinical social work position in an OMH licensed outpatient mental health clinic/CCBHC. Provides evaluations; individual, group, and/or family therapy; case management; and crisis interventions. Collaborates with families, schools, and community agencies to coordinate care; links clients to embedded care coordination, SUD services, nursing, psychiatric, and medical services. Participates in clinical meetings, documentation per standards, and ongoing supervision. Must hold a New York State license (LMHC, LCAT, LMSW/LCSW) and a Master’s degree; minimum one year of clinical experience; bilingual preference. Hybrid work arrangement: 3 days in office and 2 days remote; schedules include evenings and telehealth options.
