Job Summary
Conduct thorough evaluations and assessments of patients with various functional limitations, designing tailored treatment plans aimed at achieving specific rehabilitation goals. Effectively implement and modify treatment techniques based on each patient's unique needs and responses to therapy, using evidence-based practices. Regularly consult with other members of the healthcare team to ensure a coordinated approach to patient care, addressing any changes in conditions promptly. Document all patient care activities accurately and in a timely manner, ensuring comprehensive records that reflect progress and treatment efficacy. Lead discharge planning processes, ensuring patients and families understand post-treatment instructions and follow-up care requirements. Participate in professional development opportunities, including staff meetings and training sessions.
Required Qualifications
- A Doctorate of Physical Therapy (DPT) degree from an accredited program
- Current and valid state licensure to practice as a physical therapist in New Jersey
- At least 2 years of experience in a clinical setting, specifically in rehabilitation, with a strong focus on functional rehabilitation
- Valid CPR certification
- Excellent interpersonal skills to effectively communicate with patients, families, and team members
Desired Qualifications
- Experience in creating and utilizing digital app solutions for patient engagement and treatment tracking
- Background in team-based physical therapy practices and understanding of health care reimbursement processes
