Job Summary
Licensed Counselor or Social Worker needed at Fairfield Community Health Center to provide Behavioral Health services for youth and adults. Responsibilities include assessing strengths and needs of referred individuals and families, coordinating clinical and social services, conducting utilization review and quality assurance, maintaining documentation, participating in program evaluation and team meetings, meeting billing productivity requirements, and contributing to cross-systems training and community resource knowledge. Must meet licensure requirements defined by the Ohio board; experience with multi-need individuals and families; strong communication skills; and ability to operate in an Internet-based office environment. Required Qualifications
- Must meet requirements for licensure as defined by the Ohio Counselor, Social Worker, and Marriage & Family Therapist Board
- Experience with multi-need individuals and families
- Broad knowledge of community service systems
- Willing to participate in and lead cross-systems team-building activities
- Able to effectively communicate through verbal/written expression
- Must be able to operate in an Internet-based, automated office environment
