Job Summary
Provides direct patient care to hospice and home health patients in the patients’ home or place of residence. Assists with hygiene, vital signs, and activities of daily living under the direction of the RN; documents observations in the electronic chart; supports patient safety and comfort; travels to patient homes with flexible scheduling. Works under supervision of a Registered Nurse and collaborates with patients, families, and caregivers to deliver compassionate, high-quality care in a home-health/hospice setting.
Required Qualifications
- Education: High School Diploma or equivalent
- Virginia State Approved CNA program Certification
- Current Virginia State CNA License
- Basic Life Support
- Driver’s License
- Eligibility to work in the United States and meet Virginia state employment requirements
