Job Summary
Provide initial assessments, utilize psychotherapy to address mental health issues, and offer ongoing support to residents and their families while functioning as an active member of an interdisciplinary team. Manage your caseload, schedule, and documentation requirements, ensuring compliance with Medicare and commercial insurance plans. Work on-site within a 40-hour week to address the emotional, social, and psychological needs of older adults in a skilled nursing facility.
Required Qualifications
- Licensed Clinician (LMHC, LMFT, LISW)
- Master's degree
- Valid LISW/LMFT/LMHC license in the state for which you are applying
- Two years of counseling and therapy experience
- Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
- Experience collaborating with interdisciplinary teams
- Ability to foster open communication with the facility team and your Regional Clinical Manager
- Ability to take initiative and effectively communicate in multiple settings with multiple parties
- High level of organization
- Comfortable with the ever-changing environment of a skilled nursing facility
- Knowledge of the Medicare Benefit and commercial insurance plans
- Experience in the healthcare setting
