Job Summary
Provide initial assessments, utilize psychotherapy to address mental health issues, and offer ongoing support to residents and their families. Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners to address emotional, social, and psychological needs. Manage your caseload, schedule, and documentation while ensuring compliance with Medicare Benefit and commercial insurance plans. Work within a 40-hour week as an on-site clinical provider in a skilled nursing facility, fostering open communication with facility staff and regional managers.
Required Qualifications
- Licensed Clinician (LCPC, LMFT, LCSW)
- Master's degree
- Valid LCSW/LMFT/LCPC license in the state for which you are applying
- Two years of counseling and therapy experience
- Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
- Experience collaborating with interdisciplinary teams
- Ability to foster open communication with the facility team and your Regional Clinical Manager
- Ability to take initiative and effectively communicate in multiple settings with multiple parties
- High level of organization
- Comfortable with the ever-changing environment of a skilled nursing facility
- Knowledge of the Medicare Benefit and commercial insurance plans
- Experience in the healthcare setting
