Licensed Clinician

Licensed Clinician responsible for conducting bio-psychosocial assessments under supervision, providing group counseling and teaching coping mechanisms, developing treatment plans, leading sessions, preparing written reports and case summaries, and ensuring timely documentation in EMR. Role includes discharge planning, care coordination with referral sources and community partners, and completing tele-health visits as needed; requires strong organizational skills, confidentiality, and adherence to professional ethics.
