Job Summary
Coordinate referrals for federal, state, and city agencies while interviewing patients and families to resolve psychosocial problems interfering with medical treatment. Provide crisis intervention for acute situations, including locating emergency housing and services, and conduct home visits to assess patient environments. Develop performance improvement indicators, maintain updated community agency files, and prepare periodic statistical reports on caseloads. Serve as a liaison between patients, community resources, and staff, counseling clients on benefits such as Medicaid and Medicare. Participate in intra-departmental meetings, assist in budget preparation, and supervise social services student interns. Requires a Master's degree in Social Work, Georgia clinical licensure, and two years of casework experience.
Required Qualifications
- Master degree in Social Work from an accredited school of Social Work
- Licensed as a clinical social worker by the State of Georgia
- Two (2) years (minimum) casework experience in a home health or health care institution
- Knowledge of psychiatric and medical terminology
- Ability to make decisions during crisis situations
- Ability to lecture/instruct/supervise social services student interns
- Knowledge of agency policies and procedures relative to social work
- Knowledge of eligibility criteria for federal and state services such as Medicare, Medicaid, Presumptive Eligibility, Advanced Directives, SSI, Welfare, Veterans benefits
- Knowledge of community resources and facilities
- Knowledge of counseling techniques
- Considerable knowledge of the physical and psychological needs of individuals and families
- Considerable knowledge of cultural factors and belief systems which may affect family lifestyle
- Considerable knowledge of family dynamics
- Considerable knowledge of techniques of interviewing a variety of persons to obtain needed and confidential information
- Considerable knowledge of counseling principles and techniques
- Knowledge of guidelines for the provision of protective services
- Knowledge of current social and economic conditions
- Knowledge of available community agency resources
- Knowledge of the legal system and the procedures for legal protection of clients, as patients, and Southside Healthcare, Inc., as a health care facility
- Considerable skills in oral and written communication in a clear and concise manner
- Considerable skills in providing crisis intervention
- Considerable skills in documenting in the case record social services data, facts, observations, contacts, findings and recommendations
- Considerable skills in developing service plans
- Considerable skills in providing intensive case work services to clients
- Considerable skills in locating and coordinating specific services to meet client's needs
- Considerable skills in ready and comprehending material of varying levels of complexity
- Ability to drive a car
