Licensed Clinical Social Worker (LCSW) sought for senior care clinical roles in subacute and skilled nursing facilities across NJ, NY, PA, and MD. Role involves conducting individual and group psychotherapy for geriatric residents in facilities, developing and implementing evidence-based treatment plans, collaborating with facility staff and families, maintaining clinical documentation in an EHR, monitoring patient progress, and adjusting treatment plans as needed.
