Job Summary
Perform comprehensive evaluations of patients and families to assess social, emotional, and support needs. Set measurable objectives in conjunction with the rehab team and assist physicians in understanding significant social and emotional factors related to health problems. Instruct and counsel patients and families on treating and coping with emotional responses to illness while participating in care coordination activities. Identify and assist patients in utilizing community resources to achieve personal, career, and independent living goals. This role supports recovery for individuals with traumatic brain injury, spinal cord injury, or acute diagnoses within a continuity of care model where you serve as the primary treating therapist from evaluation to discharge.
Required Qualifications
- Current Licensed Clinical Social Worker Licensure in the state of Texas
- Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
- Current BLS (Basic Life Support) First Aid and CPR certification
- Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
- Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
