Licensed Clinical Social Worker (PCBH)

Licensed Clinical Social Worker (PCBH) at Military Medical Center (Nellis AFB, NV) under contract with a Florida-based service company. Serves as Behavioral Health Consultant in the Patient Centered Medical Home, providing brief cognitive/behavioral interventions (typically 15–30 minutes) and potentially classes (e.g., stress management, sleep enhancement, chronic pain). Works independently with a focus on medical readiness, clinical pathway development, and collaboration with primary care teams. Responsibilities include training completion and competency demonstration within defined timelines, documentation of care in the EMR, and participation in military-specific training and quality improvement activities. Duties may involve 8–10 patient encounters per day, on-site presence at the base, and providing guidance to providers in training or not fully licensed, with occasional research participation and presentations to primary care teams. Must hold an MSW from an accredited CSWE program and an active, unrestricted clinical license to practice social work in any U.S. jurisdiction.
