Job Summary
Conduct comprehensive biopsychosocial assessments and provide individual, family, and group therapy using evidence-based modalities like CBT and trauma-informed care. Develop individualized treatment plans, monitor client progress, and deliver crisis intervention and safety planning. Collaborate with physicians and case managers to coordinate whole-person care, connect clients with community resources, and facilitate warm handoffs to specialty services. Complete timely clinical documentation in the EHR while adhering to HIPAA and regulatory standards. Participate in quality improvement initiatives and contribute to educational workshops within a culturally humble environment.
Required Qualifications
- Active Licensed Clinical Social Worker (LCSW) license or Licensed Professional Counselor (LPC/LMHC)
- 3+ years of post-licensure clinical experience
- Comfort providing therapy independently within a supported care model
- Strong clinical judgment in assessment, diagnosis, and treatment planning
- Clear, professional communication and collaboration with care teams
- Comfort using telehealth platforms and clinical documentation tools
- Commitment to full-time work and continuity of care
Desired Qualifications
- 3+ years of post-licensure clinical experience (community mental health, integrated care, or similar settings preferred)
