Job Summary
Develop and implement individual treatment plans specifying type, frequency, and duration of therapy while establishing therapeutic relationships with patients, families, and significant others. Conduct psychosocial assessments, identify emotional or behavioral issues, and diagnose disorders using ICD-10 codes to address identified needs. Coordinate services with community agencies and health care providers to facilitate patient movement through the system and arrange appropriate care plans. Maintain records and statistics in the EMR system, participate in staff committees and clinical collaboratives, and ensure all projects are represented clearly on an updated Monday.com board.
Required Qualifications
- Must be bilingual (Spanish or Portuguese)
- Bachelor's Degree in related field
- Master's Degree in social work
- Three to five years as social worker in a community health care environment, medical center or other related field
- Active NJ license for Clinical Social Worker
