Job Summary
Provide home-based social services to patients managing depression, anxiety, poor nutrition, or resource limitations. Conduct patient assessments, document progress, and monitor psychosocial conditions to ensure alignment with individual care goals. Work within a multidisciplinary home health team including RNs, LPNs, and therapists to deliver counseling, crisis intervention, and community resource planning. Consult with family and caregivers on care plans while advocating for patient needs. Earn $85–$95 per visit with flexible scheduling options designed for work-life balance.
Required Qualifications
