Job Summary
Licensed Clinical Social Worker (LCSW) in Bristol, CT – full-time position within Impact Mental Health providing psychiatric consultation evaluations in skilled nursing facilities. Responsibilities include person-centered care, caseload management for psychiatric needs, coordinating care with facility teams, collaboration with medical staff, timely documentation within 24 hours, adherence to SNF regulations, and delivering assessment, diagnostic, and psychotherapy services with 1-on-1 therapy sessions. Requires active LCSW license, 2+ years of adult mental health experience, and geriatric/ SNF exposure; strong communication, assessment, diagnostic, and psychotherapy skills. Required Qualifications
- Active license in clinical social work in state of employment

- Experience in Skilled Nursing Facilities or Geriatric mental health
- Strong communication, assessment, diagnostic, and psychotherapy skills
