Job Summary
Provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities. Assess residents with consent, develop and follow treatment plans to meet needs, document treatment plans and psychotherapy sessions within policy timelines, and communicate findings to facility staff and primary care physicians. Collaborate with a multidisciplinary team, ensure medical necessity of treatment, maintain HIPAA and confidentiality, and adhere to all regulatory and policy standards. Possess knowledge of assessment and psychotherapy techniques, DSM, and use of diagnostic tools; demonstrate strong written and verbal communication, initiative, judgment, and discretion; and maintain professional relationships with care teams and facility staff. RESPONSIBILITIES include coordinating with Resident Care Coordinators, maintaining open communication, and ensuring compliance with organizational policies and regulatory guidelines while delivering high-quality clinical care to residents.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state in which you will practice (LCSW or LISW)
