Job Summary
Provide recovery-oriented rehabilitation services in group or individual settings for adults in skilled nursing and assisted living facilities as a Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW). Develop and follow treatment plans, conduct assessments, and document progress within 48 hours of service. Collaborate with facility staff, primary care physicians, and Eventus WholeHealth support staff; maintain open communication and adherence to HIPAA and regulatory guidelines; apply psychotherapy and assessment techniques, utilize DSM-based tools, and contribute to continuous quality improvement within the multidisciplinary team.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state (LCSW or LISW)
