Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) needed to provide recovery-oriented rehabilitation services in long-term care settings (skilled nursing and assisted living). Work as part of a multidisciplinary team to assess residents, develop and follow treatment plans, document psychotherapy sessions, and communicate with facility staff. Must maintain patient confidentiality, adhere to HIPAA, and demonstrate knowledge of DSM, assessment and psychotherapy techniques, and care coordination within Eventus WholeHealth’s policies. Requires Master’s degree in social work and current state license; clinical supervision and knowledge of regulatory guidelines/training are essential. Location: Muncie, IN.
