Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) sought to provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities. Collaborate within a multidisciplinary team, develop and follow treatment plans, perform consultations with primary care providers and facility staff, document psychotherapy sessions and updates, and maintain HIPAA-compliant confidentiality. Requires knowledge of DSM, diagnostic tools, psychotherapeutic interventions, and ability to communicate effectively with residents, families, and care teams. Must adhere to professional ethical standards and regulatory guidelines; regular coordination with clinical supervisor for complex cases. Responsibilities include assessing residents, facilitating treatment planning, documenting care, and ensuring medically necessary services are provided, with emphasis on quality of life improvements for a vulnerable population. Required Qualifications
