Job Summary
Assess residents in long-term care facilities to determine needs and develop individualized treatment plans; provide recovery-oriented rehabilitation services in group or individual settings; maintain documentation and ensure timely submission within facility records; collaborate with facility staff, primary care physicians, and multidisciplinary team; adhere to HIPAA, policies, and regulatory guidelines; communicate treatment plans and caregiver instructions; consult with clinical supervisor as needed; demonstrate professional integrity, strong communication, and ability to work with vulnerable populations in skilled nursing and assisted living settings.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state in which you will practice
