Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) needed to provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities. Duties include conducting resident assessments with consent, developing and following treatment plans, coordinating with primary care physicians and facility staff, documenting treatment plans and sessions, communicating needs with the Resident Care Coordinator and care teams, ensuring confidentiality, HIPAA compliance, and adhering to regulatory standards. Requires knowledge of DSM, psychotherapy techniques, outcome-focused interventions, and ability to collaborate within a multidisciplinary team in a long-term care setting. Must hold a Master’s degree in social work and an active license to practice in the state of practice.
