Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) needed to provide recovery-oriented rehabilitation services in skilled nursing and assisted living facilities. Responsibilities include assessing residents with consent, developing and following treatment plans, documenting psychotherapy sessions and updates, coordinating with facility staff and primary care providers, maintaining HIPAA-compliant confidentiality, and adhering to regulatory and organizational policies within a multidisciplinary team to support a vulnerable resident population.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state of North Carolina (LCSW or LISW)
- valid licensure for practice in NC
- knowledge of assessment techniques and psychotherapy
- DSM knowledge
- ability to develop and follow treatment plans
- HIPAA compliance
- ability to communicate effectively with facility staff and patients
- skill in documenting treatment plans and sessions
- ability to work as part of a multidisciplinary team
- professional conduct and confidentiality policies
