Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) sought to join Eventus WholeHealth in a long-term care setting. Provide recovery-oriented rehabilitation services in group or individual settings to residents in skilled nursing and assisted living facilities. Develop and follow treatment plans, perform clinical documentation, coordinate with facility staff and primary care teams, ensure HIPAA-compliant care, and maintain open communications with facility leadership. Must hold a Master’s degree in Social Work and a current state license; responsibilities include assessment, psychotherapy (group/individual), treatment planning, documentation within 48 hours, collaboration with interdisciplinary teams, adherence to policies and regulatory requirements, and maintaining professional and ethical standards. Location: Elkin, NC; on-site care delivery within skilled nursing and assisted living facilities.
Required Qualifications
