Job Summary
Provide rehabilitation-oriented clinical social work services in group or individual settings for residents of skilled nursing and assisted living facilities as an LCSW/LISW. Conduct assessments, develop and follow treatment plans, and document plans and psychotherapy sessions per policies within 48 hours. Collaborate with facility staff (Resident Care Coordinator, care team) to address residents’ needs and communicate findings. Maintain professional conduct, patient confidentiality, HIPAA compliance, and adherence to regulatory standards. Knowledge of DSM, psychotherapy techniques, and ability to utilize various diagnostic tools, with strong written/verbal communication and initiative.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state in which you will practice
- Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) credential
- knowledge of assessment techniques, psychotherapy techniques, DSM, and regulatory rules governing psychotherapy practice
- ability to maintain patient confidentiality and HIPAA compliance
- ability to work collaboratively with health care team
- ability to develop and implement treatment plans and document accordingly
- experience in long-term care settings (skilled nursing/assisted living) preferred
