Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) needed to join Eventus WholeHealth’s team providing recovery-oriented rehabilitation services in long-term care facilities (skilled nursing and assisted living). Deliver assessment-based care, develop and follow treatment plans, provide psychotherapy in group or individual settings, and coordinate with facility staff, primary care physicians, and Eventus support staff. Maintain documentation within 48 hours for the patient’s medical record, ensure HIPAA compliance, and uphold regulatory and ethical standards. Strong knowledge of DSM, assessment techniques, psychotherapy modalities, and ability to communicate effectively with multidisciplinary teams; ability to work collaboratively and handle confidential information.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state in which you will practice
- Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW)
