Job Summary
Provide recovery-oriented rehabilitation services as a Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) in long-term care facilities (skilled nursing and assisted living). Assess residents with consent to determine needs, develop and follow treatment plans, and deliver group or individual psychotherapy. Coordinate with facility staff, primary care physicians, and Eventus WholeHealth support staff; maintain documentation within 48 hours for medical records; ensure HIPAA compliance and adhere to regulatory guidelines. Requires MSW and active licensure (LCSW or LISW) in the practicing state; knowledge of DSM, assessment techniques, psychotherapy, and multidisciplinary collaboration.
Required Qualifications
- Master’s degree in social work
- current license to practice as a Social Worker in the state of practice (LCSW or LISW)
- Knowledge of assessment techniques and psychotherapy techniques
- Knowledge of DSM
- Knowledge of HIPAA rules and regulations
- Skills in gathering and analyzing data; written and verbal communication
- Ability to work collaboratively with health care team
- Ability to handle confidential information
- Ability to contribute to treatment planning and care coordination
- Experience with group or individual psychotherapy in long-term care settings
