Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) to provide recovery-oriented rehabilitation services in group or individual settings for adult residents in skilled nursing and assisted living facilities. Responsibilities include assessing residents with consent, developing and following treatment plans, documenting psychotherapy sessions and updates within 48 hours for the facility record, coordinating with facility staff and treating physicians, ensuring HIPAA compliance, maintaining patient confidentiality, and adhering to professional, ethical, and regulatory standards. Knowledge and application of DSM-based assessment, psychotherapy techniques, and a strong understanding of care delivery in long-term care settings are required. Must collaborate with a multidisciplinary team to improve quality of life for a vulnerable population and maintain effective communication with facility staff and administration.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state in which you will practice (LCSW or LISW)
