Licensed Clinical Social Worker (LCSW)

Provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities. Assess residents to determine needs, develop and follow treatment plans, and ensure services are medically necessary. Document treatment plans, psychotherapy sessions, and updates; maintain timely written documentation for the patient’s facility medical records. Collaborate with facility staff, Primary Care Physicians, and support staff; communicate findings or concerns to Resident Care Coordinators. Adhere to Eventus WholeHealth policies, HIPAA rules, and regulatory guidelines; maintain patient confidentiality. Consult with the clinical supervisor when facing unfamiliar requests. Demonstrate professional conduct, effective communication, and ability to work with a multidisciplinary health care team. All duties require knowledge of assessment and psychotherapy techniques, diagnostic tools (DSM), and compliance with applicable health care regulations.
