Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) sought to provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities. Works as part of a multidisciplinary team to address mental health issues from anxiety and depression to severe and persistent conditions; develop and follow treatment plans to meet residents’ needs; document psychotherapy sessions and treatment-plan updates; coordinate with residents’ primary care physicians, facility staff, and Eventus WholeHealth support staff; maintain patient confidentiality and comply with HIPAA; adhere to regulatory guidelines and organizational policies; conduct professional communications with facility staff and families; utilize assessment techniques and DSM knowledge to inform care; demonstrate initiative, judgment, and discretion, and ensure high-quality continuous care.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state in which you will practice
- License to practice as LCSW or LISW
