Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) to join Eventus WholeHealth in long-term care facilities. Deliver recovery-oriented rehabilitation services to adults in skilled nursing and assisted living settings, as part of a multidisciplinary team. Conduct assessments to develop and follow treatment plans, provide psychotherapy in group or individual settings, document plans and sessions, communicate with facility staff and primary care/facility teams, ensure HIPAA compliance, and adhere to regulatory and ethical standards. Requires a Master’s degree in social work and a current state license (LCSW/LISW) with knowledge of DSM, assessment techniques, psychotherapy modalities, and confidentiality; ability to collaborate with healthcare colleagues and maintain professional standards.
