Job Summary
Licensed Clinical Social Worker (LCSW) – Case Manager position at Copper River & The Native Village of Eyak for Military Treatment Facilities in Okinawa and Kadena, Japan. Provides comprehensive case management, discharge planning, crisis intervention, patient advocacy, psychosocial support, and care coordination across the continuum of services; collaborates with physicians, nurses, behavioral health professionals, and community resources; develops and monitors individualized CMPs; participates in Utilization Management and quality-improvement initiatives; delivers education and training on social work and case management topics; maintains compliant documentation.
Required Qualifications
- Master of Social Work (MSW) degree from CSWE-accredited program
- Current, full, active, and unrestricted Licensed Clinical Social Worker (LCSW) license in a U.S. state/territory/DC
- Certification as a Certified Advanced Social Work Case Manager (C-ASWCM) through NASW OR minimum of three (3) years of hospital-based social work case manager experience in the preceding four (4) years
- Experience in medical crisis counseling and discharge planning
- Knowledge of military and community support resources
- Excellent communication, counseling, and advocacy skills
- Ability to work effectively with interdisciplinary healthcare teams
- Strong organizational and documentation abilities
- Preferred: Experience supporting military treatment facilities or DoD healthcare programs
- Preferred: C-ASWCM certification
- Preferred: Experience working with complex medical and behavioral health populations
- Preferred: Familiarity with military healthcare systems and beneficiary populations
