Job Summary
Conduct comprehensive evaluations of patients and families to assess social, emotional, and support needs. Set measurable objectives with the rehab team and counsel individuals on coping strategies for illness-related responses. Assist physicians in understanding significant social factors and participate in care coordination to identify and resolve patient needs. Guide patients and families in utilizing community resources to achieve independent living goals. Perform patient moving, lifting, or transferring up to 100 pounds following safety procedures while maintaining fine motor skills and visual acuity. Requires current LCSW licensure, one year of rehabilitation experience, and BLS certification. Supports a continuum of care for individuals with brain, spinal cord, or stroke injuries by delivering services in home and community settings.
Required Qualifications
- Master's Degree in Clinical Social Work (LCSW)
- Current Licensed Clinical Social Worker Licensure in the state of practice
- Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
- Current BLS (Basic Life Support) First Aid and CPR certification
- Ability to lift 50 pounds
- Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
- Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
- Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
