Licensed Clinical Social Worker, LCSW

Provide direct mental or clinical social work services to senior adults and caregivers via telephonic and in-person home visits. Conduct comprehensive social work assessments to develop care plans, coordinate with PT and OT, and address unmet needs to help patients maintain independence and quality of life. Respond to crisis situations involving vulnerable or mentally unstable seniors while advocating for clients and promoting STARS program offerings. Maintain relationships with referring partners and build rapport with families. Requires a valid Illinois LCSW, 2-5 years of experience, and ability to work nights, weekends, and travel. Offers flexible part-time or full-time schedules with paid visits upfront and incentives for volume maintenance.
