Job Summary
Conduct behavioral health assessments, individual counseling, crisis intervention, care coordination, discharge planning, and connect patients to community resources. Provide comprehensive, patient-centered behavioral health care across inpatient and outpatient settings while building long-term relationships with patients and families. Work alongside physicians, advanced practice providers, and nursing staff in a collaborative rural healthcare team to strengthen and expand behavioral health services at the Critical Access Hospital.
Required Qualifications
- Licensed Clinical Social Worker (LCSW)
- Legally authorized to work in the United States
- Successful completion of 500-hour probationary period
- Criminal background check
- Pre-employment drug screen
- Ability to work a flexible schedule, as needed
