Job Summary
Licensed Clinical Social Worker in Home Care provides psychosocial assessments and develops plans under physician direction. Responsibilities include documenting findings, coordinating with the health team, initiating referrals, preparing social histories, educating patients and families about community resources, supervising Social Service staff when applicable, participating in care conferences, and contributing to quality improvement. Requires a MSW/CSWE-accredited program, Illinois licensure, CPR certification, and driving eligibility; part-time role (24 hours/week) based in Paris, IL.
Required Qualifications
- Master’s Degree or Doctorate from a school of Social Work accredited by the Council on Social Work Education
- Licensed in the State of Illinois
- Minimum of one year experience in a health care setting or equivalent experience
- Current CPR certification
- Licensed driver with automobile insurance and reliable transportation
