Job Summary
Conduct independent therapy and treatment planning within a supported care model using telehealth platforms and clinical documentation tools. Provide clinical assessment, diagnosis, and diagnosis, and collaborate with care teams using clear, professional communication. Maintain continuity of care through full-time work commitments. Peregrine Health partners with community health organizations to expand access to high-quality behavioral health care. The role requires an active Florida LCSW license with three years of clinical experience in community mental health or integrated care settings.
Required Qualifications
- Active Licensed Clinical Social Worker (LCSW) license
- Active and unrestricted license in Florida
- 3+ years of clinical experience
- Comfort providing therapy independently within a supported care model
- Strong clinical judgment in assessment, diagnosis, and treatment planning
- Clear, professional communication and collaboration with care teams
- Comfort using telehealth platforms and clinical documentation tools
- Commitment to full-time work and continuity of care
