Job Summary
Licensed Clinical Social Worker (LCSW/LISW) sought to provide recovery-oriented rehabilitation services in group or individual settings for adults residing in skilled nursing and assisted living facilities. Responsibilities include conducting resident assessments to determine needs, developing and following treatment plans, and ensuring all services are medically necessary. Collaborate with primary care physicians, facility staff, and Eventus WholeHealth support teams; document treatment plans and psychotherapy sessions, and provide written updates within 48 hours for the patient’s medical record. Maintain open communication with facility staff, adhere to HIPAA and regulatory guidelines, and consult with the clinical supervisor as needed. Requires Master’s in Social Work and current state licensure as LISW/LCSW; knowledge of DSM, psychotherapy techniques, and assessment methods; strong written and verbal communication skills; ability to work within a multidisciplinary team and handle confidential information.
