Job Summary
Licensed Clinical Social Worker (LCSW or LISW) sought to provide recovery-oriented rehabilitation services in group or individual settings for residents of skilled nursing and assisted living facilities. Responsibilities include assessing residents with consent, developing and following treatment plans, documenting psychotherapy sessions and updates, coordinating with facility staff and primary care physicians, communicating findings to care teams, ensuring compliance with HIPAA and facility policies, and maintaining professional conduct. The role supports a multidisciplinary team to improve quality of life for residents and requires knowledge of social work practice, assessment and psychotherapy techniques, DSM guidelines, and regulatory requirements.
Required Qualifications
- Master’s degree in social work and current license to practice in the state (LCSW/LISW)
- Knowledge of assessment techniques and psychotherapy techniques
- DSM knowledge
- HIPAA rules and regulations
- Ability to maintain patient confidentiality
- Ability to work with a multidisciplinary team
- Excellent written and verbal communication skills
