Job Summary
Provide recovery-oriented rehabilitation services as a Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) in long-term care facilities (skilled nursing and assisted living). Conduct resident assessments with valid consent, develop and follow treatment plans, and deliver psychotherapy in group or individual settings. Collaborate with a multidisciplinary team and facility staff, provide written documentation within 48 hours for medical records, ensure HIPAA and regulatory compliance, and maintain professional conduct with residents and staff. Requires Master’s degree in social work and current state license; knowledge of DSM, assessment and psychotherapy techniques, and ability to communicate effectively and maintain confidentiality.
