Job Summary
Provide recovery-oriented rehabilitation services in group or individual settings for adults in skilled nursing and assisted living facilities. Join our team as a Licensed Independent Social Worker (LISW) or Licensed Clinical Social Worker (LCSW) to develop and follow evidence-based treatment plans, document care per policies, and collaborate with facility staff, primary care physicians, and support staff. Maintain patient confidentiality and HIPAA compliance, adhere to regulatory standards, and communicate findings and care needs to the clinical team; participate in a multidisciplinary approach to improve residents' quality of life.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state (LCSW) or equivalent
- experience with assessment, psychotherapy techniques, and DSM knowledge
- ability to maintain patient confidentiality and HIPAA compliance
- strong written and verbal communication skills
- ability to work in a multidisciplinary team
- clinical supervision involvement
- professional ethical standards
