Job Summary
Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) needed to provide recovery‐oriented social work services in skilled nursing and assisted living facilities. Responsibilities include assessing residents with consent, developing and following treatment plans to meet individual needs, documenting psychotherapy sessions and updates within 48 hours, consulting with facility staff and primary care teams, maintaining confidentiality and HIPAA compliance, and collaborating with a multidisciplinary team to deliver high‐quality care. Requires a Master's degree in Social Work and a current state license (LCSW or LISW). Knowledge of assessment techniques, psychotherapy modalities, DSM, and regulatory rules for psychotherapy practice, plus skills in data gathering, evaluation, and effective written/verbal communication. Work in a long‐term care setting with a patient‐centered approach and adherence to professional standards. Signing bonus up to $5,000 offered.
